PharmD Project Survey 

Hello All, 

Below is a request from some of your P4 Peers to assist in their PharmD Project. Please take some time to complete the survey as your input is invaluable. 

Dear Potential Participant,

We are conducting a research study and invite you to participate. My name is Jaqueline Simpson, and I am a fourth-year pharmacy student at the University of Utah College of Pharmacy. My colleagues for this study are Clint Redd, PharmD Candidate 2024, Soren Wadsworth, PharmD Candidate 2023, and Gabriel Fontaine, PharmD, MBA, BCPS.

We want to invite you to participate in a survey to understand the Perceptions of Critical Care Pharmacists (CCPs) on Patient and Financial Outcomes.

• The objective of this research project is to learn more about current perceptions of CCPs. Your experiences as an employee working in healthcare are highly valuable, and your survey response can be used to help us identify if there are current gaps in knowledge about CCPs.

• If you choose to participate in this survey, it should take only 5-10 minutes of your time and can be completed when you can.

• The survey consists of various questions regarding your background, what you currently know about CCPs, and what type of things you would like to see CCPs do.

• Your participation will be voluntary and anonymous. Any responses you provide may be published as part of the research but will remain nameless, meaning that no one will be able to trace your participation back to you. We will ask questions about your professional background and experience working within a hospital setting.

• By completing the survey, you give your consent to participate in the research project.
